Read this before you trust an answer

61% of the 354 rules in here resolve to “verify with the payer” rather than a clean yes or no — 215 of them. That is the genuine state of RTM coverage in 2026, not a gap in this reference: most commercial plans simply have not published a position on these codes yet.

So treat this as a way to find out which of your payers are settled and which are ambiguous. That is a genuinely useful thing to know before you build anything — and the ambiguous ones are worth a call to the payer, not a guess.
